It is actually quite fortunate that you are so acurately able to pinpoint the cause of your tinnitis. Hearing loss and tinnitis from cochlear damage often occur when one has had any kind of surgery to the head and ears as you have had.
It is likley that you suffered cochlear damage when your right ear was surgically repaired and that the tinnitis and high frequency hearing loss was triggered by the ear wax removal.
Possibly some additional cochlear damage occurred at that time.
A little known fact is that there are literally millions of teensy-weensy nerve cells within the cochlea that are stimulated from the sometimes extreme pressures of loud noise.
When a noise is perceived by the ear and those minute hairs start moving, electrical impulses are discharged through the auditory nerve which the brain translates as sound.
This stimulation results in a stirring of the microscopic hair, similar a field of wheat on a farm rustling in time to the wind blowing.
The microscopic hair cells the microscopic auditory nerves are easy to break when you are exposed to loud noise over a period of time. The damage is all the more further aggravated if you have worked in a noisy environment for a long time.
