My tinnitus is experienced as a continuous hissing noise, although I am not aware of it all the time, when I am otherwise occupied. I have had root canal treatment, but I cannot say for certain whether this was during the two years prior to the tinnitus starting.

You did indicate to me that the possible cause of your tinnitus is an exposure to loud noises but it is also indeed probable that some damage to the delicate inner ear was experienced when the root canal treatments were done.
Most folk don't know that there are millions of diminutive cells inside the cochlea that get stimulated by the pressure of sound waves caused by external noise.
When a sound picked up by your ear and those cilia move, signals are discharged through the auditory nerve which your brain deciphers as a noise.
The stimulation then cause moving of the diminutive hair cell, similar to a wheat field on a farm moving in concert with a breeze.
All of those teeny-weeny nerve cells and those infinitesimal auditory nerve cells break very easily when exposed to sudden loud noises. This damage is all the more worsened if you have listened to loud music over an extended period of time.
Those sensitive inner ear organs like the auditory nerve cells and hairs are damaged because of a bad blow on to the head. All those minute hairs bend out of shape or even broken off and then send random impulses to the brain which are interpreted as sound, although there is no sound present.
Cochlea damage which leads to a chirping in the ears often is caused by ear wax treatments gone wrong, hopi ear candling, swimming accidents, playing sports, dental surgery or even from middle ear infections.
Hearing loss is most certainly permanent although the t-gone remedies have proved to be extremely successful at lowering the tinnitus levels that resulted from damage to the cochlea.There are thousands of little hairs housed in the inner ear that move when you perceive an external sound.

These sensitive organs can also be damaged from a knock on the head. The tiny hairs often get bent out of shape or even broken and then randomly transmit electric pulses directly to the brain which interprets them as a sound, although there is no sound present.
Tinnitus from noise (cochlea damage tinnitus) is also caused by scuba diving accidents, swimming incidents, removals of ear wax etc.
