I do believe that you have correctly determined the cause of your tinnitus to be cochlea damage. This was most likely as a result of the shock and noise of the surgery.There are millions of tiny hairs situated in the cochlea that get affected when you perceive an external sound.
As and when an external sound is detected the minute hairs move and send impulses via the auditory nerve which is interpreted as a noise by your brain.
These small hair cells and those easily damaged auditory nerves are easy to break when exposed to loud noises. The damage is all the more amplified if you have played in a band or worked in a noisy factory for a long time.
These delicate organs can also be damaged from a knock on the head. The tiny hairs often get bent out of shape or broken and then randomly send electric pulses directly to the brain which interprets them as a noise, although there is no sound present.
Tinnitus from noise (cochlea damage tinnitus) is also a result of scuba diving accidents, swimming pool incidents, ear wax removals etc.
